Release of Records:
For patients requesting a release of records, print and fill out the Request for Release of Records form, either fax/mail to your home office or email to softtouchdental@gmail.com
New Patient Paperwork:
New patients can fill out paperwork at the office or you can save time by printing and filling out our New Patient Paperwork. Please bring this with you to your first appointment.
IV Post-Op Care:
Patients undergoing IV sedation will receive an IV Sedation Packet that includes post-op care instructions.
Language Assistance:
All forms and pertinent information are available in another language upon request. Language assistance is also available, please let the receptionist know if a translator is needed when scheduling your appointment.
